Custom Reporting with a Parent-Child Realm

Please ensure you have read the prerequisites, which illustrates the permissions required for custom reporting in Sunwave.

How do I know if I have a Parent-Child Realm?
In Sunwave, click on your logo. If you see multiple realms, you have parent-child realms.

If you do not see multiple realms, you do NOT have a parent-child realm. Follow THIS GUIDE instead.

Custom Fields

Setting up Custom Fields in List of Values

Custom fields can only be done in the parent realm. This is typically your CRM realm (Realm name ex. Sunwave CRM).

  1. In the parent realm, go to Left Panel → Practice Setup → List of Values

  2. In the Header, change your Module to EMR and Entity to Custom Field

  3. Create a new registry

  4. Name the label you’re wanting to report (ex. Smoker, Legal Status, etc.) and click Create.

    1. Typically, Name and Description is the same. If you want to clarify/explain the Name field, that’s what description is for. Name is only seen in Sunwave, description is not.

  5. Once you’ve created the new Custom Field, you will see it in the list

Compatible Form Builder Fields for Custom Fields

  • Text Field

  • Date field

  • Text Area

  • Add Choice

Setting up Custom Fields in Form Builder

If you do not have access to Form Builder, you can ask Sunwave Support (support@sunwavehealth.com) to set this up in Form Builder. You will need to provide the following information to Sunwave Support:

  • Name of Form

  • Field you want Custom Field for (ex. in Section “A”, question asking “XYZ”, please add “BCD” custom field

  1. In the parent realm → Left Panel → Forms Builder, locate the form you want to add Custom Field reporting to

  2. In the form, locate/add the field you want to add Custom Field reporting to

     

  3. In the Edit Input screen, click the Reportable Field dropdown and select the Custom Field you want to attach to the field

  4. Click “Add” in the Edit Input screen and save your changes in Form Builder

  5. Copy this form across all your realms (example shown is done in the child-realm)

     

  6. Click “Add” in the Copy Form screen and save your changes in Form Builder

  7. Test the form in a test chart in both the parent- and child-realm

How to Run a Custom Field Report

  1. In the parent realm, go to Left Panel → CRM → Custom Fields Report

  2. Set your report date duration and search parameters and click the image-20241023-195953.png icon to generate your report

  3. Pull over the following fields (bubbles), or more, to see your Custom Field in action in parent and child realms

Reportable Checkboxes

Compatible Form Builder Fields for Reportable Checkboxes

  • Add Checkbox

  • Add Multiple Check Boxes

Setting up Reportable Checkboxes in Form Builder

Reportable Checkboxes does not require List of Values access

  1. In Left Panel → Form Builder → Select the form you want to add Reportable Checkboxes to

  2. In the form, locate/add the field you want to add Custom Field reporting to

  3. In the Edit Checkbox screen, you can freely type the name of the Reportable Field

  4. Click “Add” In the Edit Checkbox screen and save your changes in Form Builder

  5. Copy this form across all your realms (example shown is done in the child-realm)

  6. Click “Add” in the Copy Form screen and save your changes in Form Builder

  7. Test the form in a test chart in both the parent- and child-realm

  1. In the parent realm, go to Left Panel → CRM → Reportable Checkboxes Report
