Required Patient Data to Generate Claims
Here is a snapshot of some key pieces in the Patient’s chart that need to be entered in order for successful claim generation
Instructions
Enter Required Sections in Face Sheet:
Contact Information: Patient Address, City, State, and Zip
Demographics: Gender
Primary Subscriber Information: Relationship to Subscriber, Address, City, State, and Zip, Date of Birth, and Gender
Primary Insurance Information: Insurance Provider, Submit Claims To, Member Id
Diagnosis: Check to make sure the Dx start date is valid for the date of service
If selecting Self Pay as insurance provider, put a dash in the Member ID field
From MENU, Primary Insurance Required Sections:
After you’ve entered the required Face Sheet data, scroll back up to the top and click on Menu tab, then select the Primary Insurance
Once open, you will see a message: There is not an Insurance set for this patient. Press the plus sign to add one
Click the “+” button above the Insurance Provider table on the left side and a fly out box will open allowing you to select from the Insurance Payers that are loaded
If you have the Active Date and Term Date of the policy add both, if not you can add the current date for the plan (or prior to admission), and you can leave the Until date Blank, then hit the blue SEND at the bottom of this box
Until date is NOT Required unless there is an end date on the plan
Copy Insurance Data from Forms option:
The Patient Information and Subscriber Information sections will be blank but there will be two pieces of paper overlapping one another in the upper right hand corner.
If you hover your cursor over the overlapping dog eared pages, it will show a message: Copy Insurance Data from forms
When you click on the pages, a fly out box will populate: Confirm You have requested to copy the Insurance Information from the Patient's Chart. Do you want to continue?
With the options of Cancel, or the Blue Yes will pull all the data and populate these required sections, then select the Blue OK tab at the bottom of this screen to save.
