Objective
You want to transfer a patient from one realm to another
You need 2 charts: a treatment chart and a recovery residence chart
Steps
Locate the patient you are needing to transfer
In the chart, locate the Menu button at the top-right of the chart
Select “Refer Out” in the Menu dropdown
Complete the “Refer Out” pop-up and click “Send” when complete
Once the patient is sent, you will need to go to the other realm by clicking on the logo at the top-right
Select the respective realm the patients was referred out to
In the new realm’s left-panel, locate “Intake Process”
Select the appropriate service facility. Enter the from- and to-dates that the patient’s admission was set for. Click “Go”.
