How do I use the FoRSE integration in Sunwave?

About the FoRSE integration
FoRSE (Foundation for Recovery Science and Education) collects standardized outcomes data to improve substance use disorder (SUD) treatment. Sunwave integrates directly with FoRSE, automatically submitting completed FoRSE forms each night.
FoRSE requires the following forms for each patient:
FoRSE Demographic Information Form
FoRSE Admit Survey
FoRSE Discharge Survey

How Sunwave Supports FoRSE
Sunwave's integration with FoRSE means you can directly contribute to this national effort to improve addiction treatments. Our platform has been designed to ensure a seamless transition, making it effortless to participate in FoRSE.
Enabling FoRSE in Sunwave is a straightforward process. The platform is configured to facilitate the automatic submission of FoRSE forms. It's as simple as creating a FoRSE form within Sunwave, and the system takes care of the rest. Sunwave ensures your participation in FoRSE is smooth, efficient, and unobtrusive to your usual workflow.
Please be aware FoRSE requires the Demographic, Admit and Discharge Survey to be performed for each patient.
FoRSE Forms
FoRSE forms within the Sunwave platform are pre-created and can be quickly activated for use. Our Customer SuccessThese forms are standardized and are not editable, maintaining the integrity of the data submitted to FoRSE. Important forms such as the "FoRSE Demographic Information Form" and "FoRSE Admit Survey" need to be completed for the system to send any assessment forms and the FoRSE Discharge Survey form to FoRSE.
Step 1 -Create FoRSE forms
Go to Forms Builder and click + NEW FORM. Check the box labeled "Is FoRSE Form", make sure to check this.
Once you’ve checked the box labeled "Is FoRSE Form" a dropdown will appear containing all the FoRSE forms.
Select the desired FoRSE form from the dropdown. These forms are pre-created and cannot be edited.
Review and adjust flags as needed:
Is included in the patient chart
Multiple Instances Per Admission (n-Form) - only for assessment forms (not for Demographics, Admit, or Discharge)
Include in Patient Engagement (if enabled in your system)
Published
Include signature Section > User Signature
Can be locked
Enter a sequence number. We recommend placing the Demographic and Admit forms first.
Click Add to create the form.
Note: FoRSE does not require patient signatures or required fields.
Step 2 - Adding FoRSE Forms to Intake Process and Assigning Permissions
2a. Add forms to Form Sets
Navigate to Form Sets from the left menu.
Select the module (EMR, CRM, or Alumni) where you want to include the forms.
Drag and drop the FoRSE forms from Available Forms into Forms in Set.
For intake, always include the Demographic and Admit forms. These must be completed before any other FoRSE forms can be sent.
2b. Assign permissions
Go to Practice Setup > Roles.
Select the role that should access FoRSE forms.
Scroll to the FoRSE forms section.
Change from No Access to Read Only or Read and Write.
Step 3: Access FoRSE Reports
You can track the completion and submission of FoRSE forms through Sunwave's built-in reports. These reports provide you with an overview of which forms have been completed, if they've been sent to FoRSE, or if they're on hold. Moreover, the reports also contain detailed information about each form, including which patient the form was completed for, the name of the form, the service facility, the service date, when it was sent (if sent), when it was last updated, and the contents of the form (the answers).
It's worth noting that FoRSE forms are sent nightly automatically, so there's no additional action required from you to send the completed forms to FoRSE.
Ensure the user has the Access FoRSE Report permission (Practice Setup > Users).
Go to Reports > FoRSE Reports.
Use the filters above the table to search by Service Date, Sent On Date, or Service Facility.
Review the report table for form completion, submission status, and details such as patient name, form name, facility, service date, and submission status.
If a form shows On Hold, it means the Demographics or Admit form is incomplete. Once completed, the held forms will be sent automatically the next night.
Notes
Forms are sent to FoRSE nightly; no manual send action is required.
A reporting feature for all forms is planned, similar to the Assessment Score Report.

If you need assistance with the FoRSE integration, contact Sunwave Support or your Customer Success representative.As mentioned before, both of these forms MUST be completed for any other forms to be sent. Once these are completed, the forms that are on Hold will be sent that night, so the User can check the next day to confirm that.
